Patient Care Coordinator

Care Team Management:

• Act as primary point of contact for patient care team communication.
• Triages and resolves clinical messages, relays information between patients and clinicians.
• Conducts regular outgoing communications to ensure patient engagement.

Lab / Medical Records Support:

• Collect and review medical records from external providers.
• Facilitate lab requisitions and prior authorization coordination.
• Support clinicians with pre-visit lab orders.

Membership / Program Status:

• Guide new patients through program expectations and process.
• Address general program questions and assist with account changes.
• Provide scheduling and appointment support.

Billing / Insurance:

• Assist with insurance and billing inquiries.
• Provide information on coverage and membership costs.
• Coordinate with billing department to resolve concerns.
