About the Role:
- Manage a portfolio of clients as they navigate the healthcare system, providing education, advocacy, and care coordination.
- Support the Triage Team to welcome and educate new PHM patients, placing them with appropriate internal teams.
What You'll Do:
- Build trusted relationships and serve as a reliable guide throughout each client's healthcare journey.
- Conduct clinical intakes via email or phone and collaborate with internal teams to streamline care.
Compensation & Benefits:
- Target base salary $85,000–$100,000 annually, plus health, dental, and vision benefits.
