Primary Duties:
- Provide education and self-management support, coordinate care, and conduct comprehensive assessments.
- Collaborate with primary care physicians and interdisciplinary teams to design care plans.
- Measure quality outcomes and support population health initiatives.
Minimum Qualifications:
- Active Michigan license as RN or LMSW.
- Resident of Michigan.
- 3-5 years of direct healthcare experience in case management or care coordination.
