Responsibilities:
- Assess member needs in physical health, mental health, SUD, oral health, palliative care, and social supports.
- Develop client care plans, set SMART goals, and evaluate progress using evidence-based practices like Motivational Interviewing.
- Conduct outreach, arrange transportation, and maintain accurate documentation in EMR and CRM systems.
Qualifications:
- 2+ years of experience as a care manager or community health worker.
- Working knowledge of government and community resources.
- Excellent communication skills, computer proficiency, and a valid driver's license.
Work Environment:
- Remote position with required availability Monday-Friday 8:30am-5:00pm Pacific Time.
- Occasional travel may be required; work involves sitting, computer use, and lifting up to 25 pounds.
