Responsibilities:
- Analyze health plan reports and clinical data to identify members eligible for case management.
- Conduct telephonic and virtual assessments and develop individualized care plans.
- Coordinate healthcare services across the continuum of care and facilitate care transitions.
Qualifications:
- Current unrestricted RN license required; BSN preferred.
- Minimum two years clinical nursing experience; case management experience preferred.
- Strong communication, assessment, and critical thinking skills; proficiency with EMR systems.
