What You’ll Do:
- Provides active care management and coordinates action plans for members.
- Evaluates outcomes and ensures accurate documentation of clinical information.
- Serves as member advocate and promotes enrollment in care management programs.
Qualifications:
- Requires an active RN license or social worker license and 4 years clinical experience.
- Must have working knowledge of Microsoft Office and word processing.
- Ability to work independently, prioritize, and make sound decisions.
