Role Responsibilities:
- Engage patients through various communication channels to facilitate program engagement and follow-up.
- Update patient records in EHRs and maintain a collaborative care registry for tracking outcomes.
- Facilitate outside referrals for community-based social services as needed.
Qualifications:
- Required Bachelor’s or Master’s degree in nursing, social work, or psychology; clinical licensure not required.
- Strong understanding of women’s mental health conditions and community resources.
- Proficiency in EHR systems and Google Suite.

- Strong communication and organizational skills for effective patient and provider interactions.
