Care Navigation Specialist

Position Overview:

• This role combines direct patient and caregiver support with flexible Care Navigation and essential clinical operations responsibilities.
• You will be trained in our Care Navigation model and support patients and caregivers through outreach, resource navigation, and GUIDE check-ins.
• You will also support day-to-day administrative workflows that keep patient care moving.

Core Responsibilities:

• Serve as a knowledgeable point of contact for patients and family caregivers, using care navigation skills to connect them with appropriate services.
• Support appointment scheduling, new patient intake, onboarding, and documentation follow-up.
• Accurately document patient interactions and escalate concerns to licensed clinicians when needed.

Qualifications:

• Bachelor's degree in Social Work or a related human-services field required; MSW preferred.
• Experience supporting patients or caregivers navigating health and social needs.
• Strong communication, organizational, and problem-solving skills with ability to work in a fast-paced environment.
