Patient Support:
- Address patient and durable power of attorney inquiries via telephone and respond to messages through the patient portal.
- Assist patients with scheduling follow-up appointments with Curana providers or specialists.
Provider Support:
- Manage electronic health records and ensure accurate patient records, coordinate documents for provider review or signature.
- Facilitate provider orders, escalate findings, maintain patient rosters for APCM and GUIDE programs, and assist with prior authorizations.
Communication Support:
- Answer and manage incoming calls professionally and courteously.
- Collaborate with the interdisciplinary care team to ensure seamless communication across the health network.
