About the Role:
- You become the patient's go-to support after their initial visit with a clinical lead.
- You help them understand care plans, schedule appointments, and navigate insurance.
- You remove friction that keeps patients from getting the care they're entitled to, without giving medical advice.
Who You Are:
- You genuinely care about older adults and people facing barriers to care.
- You're organized and self-directed, able to prioritize across a full caseload.
- You're comfortable with technology and learn new tools quickly.
What You'll Do:
- Serve as the primary point of contact for patients after their intake visit.
- Help patients understand and act on their care plan, including scheduling and finding providers.
- Document every patient interaction accurately and on time.
