About the Role:
- Serve as frontline support for seniors navigating social and clinical situations.
- Address social determinants of health, secure transportation, and coordinate food assistance.
- Help patients manage chronic conditions by improving health literacy.
Role Logistics:
- This is a 1099 contract role, fully remote, based in the United States.
- Requires a baseline capacity of 25+ hours per week, with 30+ hours prioritized.
- Must be available during standard business hours to coordinate with clinics and insurance companies.
What You'll Bring:
- Licensure: California Registered Nurse (RN) license in good standing.
