About the Role:
- You'll coordinate care for members beyond the digital space, driving behavior change and improved outcomes.
- You'll work remotely in a 24/7/365 environment, with shifts available across different time zones.
What You’ll Do:
- Clearly communicate the value of Maven's care coaching model to optimize program engagement.
- Co-create SMART goals with members and track adherence to care plans.
- Manage a caseload through messaging, video appointments, and outreach.
What You’ll Bring:
- 1-2+ years of health coaching or clinical experience with focus on patient education and motivational interviewing.
- Demonstrated experience with maternity population and family planning.
- Ability to quickly learn digital health tools and embrace AI to enhance member outcomes.
