Patient Engagement Specialist

The Role:

• We're hiring a Patient Engagement Specialist to own proactive outreach that keeps patients active and adherent in Cadence's remote monitoring program.
• This role sits between patients and their care team, driving appointment scheduling, supporting consistent monitoring, and re-engaging with patients.
• Success means patients stay connected to care between visits, not just during them.

What You'll Do:

• Own proactive outbound outreach to schedule patient appointments with Cadence clinicians and care team members.
• Drive patient adherence to daily vital sign monitoring through consistent, personalized engagement.
• Identify patients at risk of disengaging and execute re-engagement outreach to restore active participation.

What You Need:

• 2+ years of experience in sales, customer service, patient outreach, or healthcare support roles.
• Excellent verbal and written communication skills, with the ability to build rapport and resolve objections.
• Proficiency with CRM platforms and AI productivity tools in a fast-paced, metrics-driven environment.
