About the Role:

  • The Lead Care Manager (LCM) serves as the primary coordinator for clients, partnering with healthcare providers, specialists, and social service agencies.
  • This role manages client cases, coordinates health care benefits, and facilitates access to care in a timely and cost-effective manner.

What You'll Do:

  • Assess member needs across physical health, mental health, SUD, oral health, and social supports.
  • Develop and evaluate client care plans and SMART goals while utilizing evidence-based practices.
  • Conduct outreach, document in EMR and CRM systems, and complete monthly reporting for program compliance.

Qualifications:

  • Minimum 2 years experience as a care manager, navigator, or community health worker.
  • Working knowledge of government and community resources, with skills in Google Workspace and MS Office.
  • Must be willing to work Monday-Friday 8:30am-5:00pm Pacific Time, with flexibility for evenings/weekends.
