Health Coach

What you will do:

• Conduct telephonic outreach to Medicare members to improve medication adherence and HEDIS diabetes and blood pressure control.
• Use motivational interviewing to assess health risks and engage members in behavior change.
• Partner with members to identify opportunities for integration into internal/external programs and coordinate care.

Required Qualifications:

• Residence within the Eastern or Central Time Zone.
• 3-5 years of clinical or practice experience in Renal, Diabetes, Hypertension, Primary Care, Geriatrics, or Oncology.
• Experience supporting Managed Care and Medicare populations.
• 1-3 years of experience using Microsoft Office applications.
• Reliable hardwired internet connection.
