This position is for a full-time Lead Care Manager to play a critical role in Pair Team's whole-person, interdisciplinary care model, responsible for directly outreaching and engaging with individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. The position primarily allows for remote work; however, it includes 1-2 times a month on-site visits in the community.
The National Rehabilitation Consultant (NRC) provides support to WorkFocus Australia teams across the country, using skills as a Rehabilitation Consultant and knowledge of different state schemes. Responsibilities include undertaking remote vocational assessments, identifying RTW barriers, liaising with clients, and designing programs to assist injured employees return to the workforce.
