Key Responsibilities:
- Act as point-of-contact to assist individuals in living in their home and/or community.
- Assist with linkages to service providers and community resources.
- Identify barriers to healthcare compliance and create solutions.
Qualifications:
- At least 18 years old with a High School Diploma or higher.
- Minimum 12 months of experience in healthcare, behavioral health, or social services.
- Experience advocating for individuals with chronic illness, mental health, substance use disorders, or physical disabilities.
