Responsibilities:
- Contact eligible patients for the Care at Home program and provide wellness coaching to reduce high-risk behaviors.
- Encourage adoption of habits for higher quality of life and deliver high patient service.
- Coordinate care with team members and vendors for optimal patient care.
Qualifications:
- Minimum 1+ year experience as a wellness or health coach or educator; senior care experience preferred.
- High School Diploma or GED required; Associate's degree or two years experience in lieu; Bachelor's preferred.
- Strong knowledge of diseases, lifestyle topics, nutrition, and exercise; ability to work independently and remotely.
Work Environment:
- Remote field with home and virtual visits; requires effective communication and flexibility.
- Regular talking, hearing, standing, walking, sitting; light lifting up to 10 pounds.
- Must maintain confidentiality per HIPAA and work with protected health information.
