Health Coaching Responsibilities:
- Conduct 30-minute, personalized 1-on-1 coaching sessions using motivational interviewing and evidence-based strategies.
- Facilitate group coaching sessions and workshops to support member engagement.
- Provide ongoing accountability, evaluate progress, and adapt strategies for long-term success.
Care Team Collaboration & Member Advocacy:
- Serve as subject matter expert in behavior change and support the multidisciplinary care team.
- Understand member health conditions and guide them to appropriate care and resources.
- Connect members with digital health tools, health plan benefits, and community resources.
Operations, Administration & Compliance:
- Manage scheduling, appointment coordination, and follow-ups for an active member panel.
- Strictly adhere to NBHWC Code of Ethics, HIPAA, PHI/PII protection, and IT security standards.
- Participate in quality assurance reviews and continuous feedback to elevate service standards.
