About the Role:
- Liaise between patients with chronic conditions and the healthcare system.
- Ensure comprehensive, coordinated care and empower informed health decisions.
Qualifications:
- Active LPN, LVN, or RN license with 2+ years clinical experience.
- Strong knowledge of care coordination, patient advocacy, and social determinants of health.
Working Conditions:
- Fully remote, 25-30 hours a week between 10am and 4pm ET.
- 1099 Contractor position with flexible schedule.
