About the Role:

• As an RN Advocate, you'll guide patients through complex health concerns and help them achieve wellness goals.
• You'll address Social Determinants of Health and work to actually resolve patient problems.
• This is a 1099 contract position, 100% remote, with flexible hours.

What You’ll Do:

• Learn our proprietary system while providing your unique experience.
• Build strong, trusting relationships with patients.
• Develop comprehensive patient care plans that address food, transportation, and support needs.

What You Bring to the Table:

• An active RN license in good standing and experience with diverse patient populations.
• Empathy, clinical skills, and a bias toward action.
• Ability to learn new systems quickly and work under pressure.
