About the Role:
- Seeking experienced RN Healthcare Advocates for older adults.
- Provide empathetic guidance and tackle complex care challenges.
- This is a remote, 1099 position with flexible hours starting at 15 hours per week.
What You’ll Do:
- Build trusted relationships with older adults and families, grounded in empathy.
- Serve as a go-to resource for medical systems, social services, and emotional wellness.
- Create individualized care plans addressing clinical needs and Social Determinants of Health.
What We’re Looking For:
- Active RN license in good standing.
- Experience in healthcare advocacy or care navigation with Medicare populations.
- Familiarity with SDOH and clinical judgment balanced with empathy.
