Hospice Admission Coordinator, RN or LVN/LPN

The Admission Coordinator will be responsible for regional coordination of hospice patient referrals, working closely with branch staff to ensure adequate coordination and transition to hospice services in a fully remote position, working Friday at 5 pm - Monday at 8 am.

Provide remote supervision over group of RN’s, performing chart audits and reviewing adherence to company policies. Assess and document the health status of new patients.
