Responsibilities:
- Report to the Supervisor of Care Management.
- Provide case management to members including medical, social, psychological, physical, and spiritual needs.
- Develop, implement, and monitor care plans in conjunction with PCP, caregivers, and other team members.
Qualifications:
- Associate’s Degree in Nursing for experienced candidates; BSN for new graduates.
- NYS RN license required; CCM preferred.
- 1 year of RN experience in managed care, home care, acute care, or nursing home preferred.
Compensation:
- Min USD $90,000.00/Yr. Max USD $110,000.00/Yr.
