Your Role:
- Manage an assigned patient panel with clinical expertise and value-based care principles.
- Build trusting relationships and provide emotional support throughout the care journey.
- Conduct comprehensive assessments including social determinants of health.
Qualifications:
- Associate Degree in Nursing and current RN license required.
- 3+ years of care management experience, chronic and complex care.
- 2+ years of Transitions of Care experience preferred.
Compensation & Benefits:
- Pay range $90,000 to $103,000 annually, plus quarterly bonus.
- 4 weeks PTO, 12 paid holidays, medical/dental/vision, HSA, 401(k) match.
