Your Role:

  • Conduct monthly clinical check-ins and build relationships with patients.
  • Perform clinical assessments and escalate concerns.

Qualifications:

  • Active LPN/LVN license and bilingual Spanish/English.
  • Minimum 1-2 years clinical experience in ambulatory care or care coordination.
  • Strong clinical assessment and communication skills.
