Your Role:

You will support proactive care coordination for patients with complex chronic conditions.
You will conduct patient outreach and manage a high-volume patient panel.
You will ensure documentation compliance and coordinate communication between patients and providers.

Role Responsibilities:

Perform patient enrollment and eligibility verification for CCM programs.
Conduct text and telephonic outreach to enrolled patients weekly.
Document patient interactions accurately and maintain compliance with Medicare billing.
