Pair Team is building a team of deeply passionate individuals ready to change primary care operations for those who need it most. The Behavioral Health Care Manager will work in a team-based model with a lead care manager, community health worker, registered nurse care manager, and nurse practitioner to contribute their expertise towards improving the individualβs quality of life through evidence-based behavioral interventions.
This position primarily allows for remote work; however, it includes 1-2 times a month on-site visits in the community alongside a fellow PairMate. You can expect to engage in these in-person activities 1-2 times per month, close to your city, while the majority of your duties, approximately 90%, will be performed from your home. The Behavioral Health Care Manager is expected to primarily work with individuals who struggle with severe mental illness or substance use disorders, use evidence-based approaches to provide short-term interventions, provide brief crisis intervention techniques and educate individuals on behavioral health conditions to improve health literacy.
