Support a high-need Medicaid population through proactive, whole-person care as a Bilingual RN Care Manager. The role involves delivering integrated medical, behavioral, and social support to individuals with complex needs and serving as the primary point of contact for a panel of high-risk members, contributing clinical expertise to build trust, address health disparities, and coordinate care across settings.
This role supports a high-need Medicaid population through whole-person, relationship-based care as a key part of Pair Team’s Care Management Program, which delivers coordinated medical, behavioral, and social services to individuals with complex health needs. The CHW will engage members who may be navigating housing instability, chronic health conditions, or behavioral health challenges—helping them stay connected to care, build trust in the system, and meet their personal health goals.
Pair Team is building a team of deeply passionate individuals ready to change primary care operations for those who need it most, seeking a full-time Lead Care Manager to play a critical role in their whole-person, interdisciplinary care model, responsible for directly outreaching and engaging with individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs.
The Credentialing Specialist supports Pair Team’s growing central operations team by owning the end-to-end credentialing and re-credentialing process for providers and facilities. This includes maintaining up-to-date records, tracking licensure and certifications, submitting applications to health plans, and supporting audits to ensure full compliance with all regulatory and contractual requirements.
