Join our dedicated team of Care Managers that specialize in providing comprehensive care for aging adults. Our team works from home and provides chronic care management services for patients who are living in skilled nursing, assisted living, and other long-term care facilities. Essential duties include performing comprehensive chart reviews, reconciling medications, and identifying potential issues. You should develop and implement personalized care plans tailored to each patient's needs, monitoring and assessing patient progress, adjusting care plans as necessary to ensure optimal outcomes. You will collaborate with other healthcare providers and facilities to coordinate patient care and ensure a cohesive approach, documenting all patient interactions, assessments, and care plans accurately in the electronic health record system and ensuring compliance with all healthcare regulations and standards of practice.
