Job Description

Identify eligible patients for palliative services in collaboration with housecalls primary care team and conduct patient and caregiver education on the goals and scope of palliative care. The role involves performing intake assessments and supporting appropriate documentation for transitions, along with collaborating with clinical teams, case managers, and community providers to ensure timely and appropriate transition to hospice when indicated. Maintaining accurate documentation in EMRs and referral systems and participating in interdisciplinary team meetings and case conferences are required.
