Identify eligible patients for palliative services in collaboration with housecalls primary care team. Conduct patient and caregiver education on the goals and scope of palliative care. Perform intake assessments and support appropriate documentation for transitions. Collaborate with clinical teams, case managers, and community providers. Ensure timely and appropriate transition to hospice when indicated.

The nutrition educator will collaborate with the member/patient to explore the nutrition-related issues and topics that are most important to the member/patient.
The Diet Technician will provide telephonic nutrition assessments to members beginning a Medically Tailored Meals program. Assessments are intended to gauge the appropriateness of Medically Tailored Meals for the individual based on their unique health condition and recommend the appropriate meal type accordingly. Utilizing a member-centric approach, the Diet Technician will collaborate with the member to understand their chronic conditions. The Diet Technician may also identify other member needs and connect them to resources available.
