Assist patients with overcoming physical, cognitive, and emotional barriers to maximize their independence in daily activities. Conduct comprehensive in-home assessments to evaluate the physical, cognitive, and psychosocial abilities of clients. Develop individualized care plans with direct input from clients and their families. Work closely with physicians, nurses, social workers, and other professionals to ensure the best outcomes for clients.
