ProSidian is seeking a Certified Yoga Instructor/Facilitator for On-Site And Virtual Wellness & Well-being Services on a Exempt 1099 Contract, focusing on Human Capital Solutions for clients such as Department of Veteran Affairs (VA) | Veterans Health Administration (VHA).
Identify eligible patients for palliative services in collaboration with housecalls primary care team. Conduct patient and caregiver education on the goals and scope of palliative care. Perform intake assessments and support appropriate documentation for transitions. Collaborate with clinical teams, case managers, and community providers. Ensure timely and appropriate transition to hospice when indicated.
As a Renal Service Coordinator (RSC), you will play a key role in assisting patients to access essential medical services and connect individuals to vital community resources through each stage of Chronic Kidney Disease (CKD), collaborating with patients, care teams, insurance providers, physicians, community resources, and vendors to improve health outcomes.
