Role Responsibilities:
- Own performance across CKD Navigation, PAC, ECCP, Pharmacy, and future clinical programs
- Ensure clinical programs align with value-based care contract requirements, quality goals, utilization targets
- Lead program integration, stabilization, and scaling across markets and payer partnerships
Required Qualifications:
- Licensed clinician with experience in direct patient care and clinical operations
- Experience leading clinical operations in value-based care, provider enablement, population health, or complex chronic care models
- Strong working knowledge of HEDIS, Stars, quality performance, utilization management, and value-based care economics
Preferred Qualifications:
- Experience leading clinical operations in a Nephrology setting
- Experience in a clinically integrated mode
- Bachelor's degree
