What you'll do:
- Remind patients to record their vitals
- Respond to alerts related to patient symptoms or physiological data
- Respond to patients for minor technical troubleshooting and provide ongoing chronic care management
What you'll need:
- Licensed Practical Nurse (LPN) with a valid nursing license in the state of practice
- Experience working in a healthcare setting, preferably in remote patient monitoring or telehealth
- Excellent communication and interpersonal skills
