Director, Community Health Center Performance

The Director will be responsible for developing and maintaining data systems to track contract performance for our CHC partners to support high quality and equitable care for the patient populations seen at CHCs. Using this data system, the Director will support performance management of CHC partners in Aledade ACO networks by surfacing opportunities for improvement based on data.The Director of CHC Performance will understand the environment and complexities under which CHCs provide value-based care (stakeholders, policies), and will be able to lead the development of new programs and resources for CHC performance improvement based on data analytics and his/her comprehensive understanding of CHCs. This role works with both internal and external partners, and requires strong organizational, communication, and analytical skills.
