Job Description
This is a full time role where you will be working 9am-5pm in your local time zone. The Patient Advocate will serve as the ongoing point of contact for Medicare patients after their intake appointment. You will educate patients on how to access healthcare services (e.g., finding providers, using insurance, scheduling appointments), troubleshoot issues related to online portals, referrals, or provider communication and collaborate with NP and MDs to support care planning based on Social Determinants of Health (SDOH). The Patient Advocate will also document clearly and update care plans in the internal systems and provide warm, empathetic, and culturally responsive support to every patient.
