Treatment Plans:
- Determine individualized treatment plans for patients.
- Facilitate changes to treatment plans when patients are not improving as expected.
- Consult with more experienced providers, when needed.
Behavioral Interventions:
- Use motivational interviewing and behavioral activation.
- Motivate patients to remain in care and adhere to their treatment plans.
- Conduct behavioral health / psych evals.
Monitor/Medication Management:
- Systematically track treatment response and monitor patients for changes in clinical symptoms and side effects.
- Monitor treatment adherence and the effectiveness of treatment.
- Prescribe medications.
