As a Care Coordinator, you will act as the patient concierge for telehealth, meeting patients, exploring their goals, matching them with the right Bold provider, answering questions about cost and coverage, and managing their appointments. You’ll help ensure a seamless, empathetic, and patient-centered experience for patients from first contact through ongoing care. You will serve as the telehealth concierge / first point of contact for patients by conducting a high volume of intake appointments (via phone, video) to understand patient goals, complete consent forms, and answer questions about service and coverage. You will also match patients with the most appropriate Bold physician or care pathway, provide clear, empathetic guidance on health plan benefits and coverage limitations, and book, confirm, reschedule, cancel, and follow up on telehealth appointments. You will also conduct outreach to patients to prompt initiation of care, and troubleshoot patient issues.
