Job Description

Assist patients with overcoming physical, cognitive, and emotional barriers to maximize their independence in daily activities. Conduct comprehensive in-home assessments to evaluate the physical, cognitive, and psychosocial abilities of clients. Develop individualized care plans with direct input from clients and their families to provide input and direction on the frequency and duration of needed OT interventions to meet client goals. Work closely with physicians, nurses, social workers, and other professionals to ensure the best outcomes for clients. Assess and train clients and families on the use of adaptive/assistive equipment, safety equipment, mobility aids, and environmental modifications. Maintain accurate documentation, including assessments, treatment plans, progress notes, and discharge summaries.
