Vice President of Care Transformation

The Vice President of Care Transformation is a razor-sharp, healthcare expert with experience leading teams. This role involves running large clinical operations and supporting partner organizations to achieve enterprise excellence. Using IMPaCT’s evidence-based infrastructure, the team aids customer organizations in planning, building, and supporting Community Health Worker programs at scale. Key responsibilities include overseeing strategic planning conversations, conducting gap analyses of existing programs, defining key performance metrics, building financial models, identifying priority populations, and mapping workflows. The VP will also help clients execute plans, hire and train Community Health Workers, optimize the use of IMPaCT’s cloud-based application, including electronic health record integration and billing pathways, ensure the client’s Community Health Worker program delivers consistent results, and manage a growing team.
