Key Responsibilities:
- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments.
- Partner with physicians and specialists to order individualized care plans under a Collaborative Practice Agreement (CPA).
- Monitor therapy effectiveness and safety, and support quality-improvement and REMS initiatives.
Qualifications and Skills:
- Must hold a Doctor of Pharmacy (PharmD) and be a licensed pharmacist in California.
- Must have completed a PGY2 Oncology Residency, or a PGY1 Ambulatory Care Residency with 3+ years of oncology experience, or 3+ years as a clinical oncology pharmacist with 1 year under a CPA.
- Requires strong communication, organization, tech skills, empathy, integrity, and a team-first mindset.
Compensation and Benefits:
- Salary range of $145,000 to $180,000 per year, plus a potential bonus and a $10,000 sign-on bonus.
