Proactively identify strategic growth initiatives and drive their implementation. Lead site operations at health system partners including overseeing management of in-clinic pharmacy liaison team. Meet or exceed both internal and external financial, operational, and patient outcome & satisfaction targets.

Communicate with patients telephonically regarding the Shields Partner Pharmacy program. Access patient EMR to review chart notes prior to outbound calls. Identify medications requiring special storage and necessary supplies.

Provide medication management for individuals, including reconciliation and adherence counseling. Collaborate with the interdisciplinary care team to develop individualized care plans. Monitor and evaluate medication regimens to ensure clinical criteria are met.

Ensures payments for pharmacy partners are retrieved and posted accurately and timely. Resolves missing remittance files and any unmatched/unreconciled remittance detail. Contributes to the month-end reporting process by retrieving necessary reporting related to payment activity.

Provide service for inbound pharmacy phone calls and/or placing outbound patient refill calls. Assist the patient through the entire refill process. Respond to patient requests and inquiries regarding prescription drug plan coverage and fulfillment through the pharmacy.
