Job Duties:
- Assure the provision of safe, efficacious, and cost-effective drug therapy.
- Perform phone, virtual or video-based assessments on disease state management and drug therapy education.
- Identify, resolve, and prevent medication-related problems.
Experience/ Education:
- Applicant must hold a Doctor of Pharmacy (PharmD) and be a licensed pharmacist in CA.
- Completion of PGY1/PGY2 residency is preferred, but not required
- Required 3+ years' experience as a clinical pharmacist.
