Pharmacy Liaison

Job Purpose:

• Provides integrated off-site support to patients, pharmacists, and providers.
• Works closely with patients to deliver medication adherence support.
• Encourages patients to utilize the hospital partner pharmacy.

Job Duties:

• Provide medication management services as an off-site clinic team member.
• Educate prospective pharmacy patients and handle their medication needs.
• Resolve insurance-related issues and secure financial assistance.

Skills:

• Strong interpersonal communication and phone skills.
• Proficient in Microsoft Office, especially Excel and Word.
• Strong organizational skills and ability to learn new programs.
