Job Description
The Clinical Pharmacist will review patient care notes, develop clinical protocols, and monitor patient adherence across various sites and disease categories.
- Review clinician's patient assessments for data needed for prior authorization approvals.
- Develop disease and drug specific clinical protocol to maximize patient adherence.
- Monitor on-going patient adherence across all sites and disease categories with focus on oncology.
The Clinical Pharmacist will evaluate EMR data, recommend drug regimens, and manage medication therapy.
- Evaluate EMR to review drug and disease interaction and medication selection.
- Recommend alternative therapeutic drug regimens and disease specific treatments.
- Manage medication therapy including reviewing and monitoring medication regimens.
The Clinical Pharmacist will oversee tech activities, address insurance issues, and maintain compliance.
- Oversee tech's daily activities and verification for accuracy.
- Identify and resolve insurance related issues.
- Supervising communication of support personnel compliant with HIPPA rules.
