QA/Documentation Specialist

The QA/Documentation Specialist is responsible for ensuring the accuracy, completeness, and regulatory compliance of all clinical documentation across the organization. This non-clinical role focuses on supporting chart reviews, obtaining care plans, and gathering documentation needed to establish medical necessity, all in alignment with Medicare/Medicaid requirements, state program rules, and accrediting body standards. Working under the direction of our Ohio-licensed clinical team, the specialist helps maintain high-quality documentation integrity without performing direct patient care or independent clinical decision-making. Key responsibilities include regulatory compliance and documentation review, ensuring documentation meets Medicare and Medicaid COP, accrediting body recommendations, and state program rules. The specialist will conduct thorough chart reviews to ensure accuracy, completeness, and compliance at all touchpoints of care, verifying that all required documentation elements are present, timely, and consistent with internal policies and regulatory standards.
