Job Description
As a Care Navigator, your responsibilities include using evidence-based interventions to skillfully manage client crises, providing brief risk assessment, safety planning, and de-escalation to connect clients to treatment. You will identify roadblocks to clinical care options, using Lyraโs proprietary software to connect clients to the ideal treatment. You will independently manage a caseload of complex, multi-need clients.
To qualify, you must have a Masterโs degree in clinical social work (or equivalent) and an LCSW (or equivalent independent clinical licensure) or a Bachelor's degree in nursing (RN). Two years of psychiatric intake, assessment, or related experience is required. EAP/clinical call center experience is preferred. Spanish-speaking proficiency is required, as well as experience managing high-risk clinical situations. You must be a full time resident of the United States.
