Scope of Work:
- Guide participants through video calls and group education in a 3-month online program.
- Develop tailored treatment plans for exercise, medication, and risk factor management.
- Coordinate remotely with Medical Directors for plan approval and share reports with providers.
Minimum Qualifications:
- Bachelor's degree in exercise science or nursing with 1-5 years in cardiac care management.
- Required certifications: ACSM-CEP for Exercise Physiologists or CCRP for Registered Nurses.
- Ability to work 15-20+ hours per week on a set schedule for a minimum of 3 months.
